Look up Hot Player in the App Store
On the Apple TV open the App Store, go to Search and type Hot Player. Free to download with a trial included; the playlist licence is an in-app purchase that also covers iPhone, iPad and Mac.

Install, then launch Hot Player
Press Get, let it finish, then start the app.

HotPlayer wants a playlist
First run shows no playlist plus a device ID and the address hotplayer.app/upload. Park that screen — you come back to it.

Attach the playlist at hotplayer.app/upload
From any browser, load hotplayer.app/upload. Pick Xtream Code as the source, drop in the server URL, username and password from your email, then save.

A phone, tablet or computer is needed here: the upload page lives on the web, not inside the app.

Hit Refresh in the app
Head back to HotPlayer and press Refresh. It reads the playlist you just attached.

You are live
Channels, films and series populate. Hit play on anything to confirm the feed.

Installing HotPlayer on Apple TV: what is different
Apple TV runs tvOS. Open the App Store from the Home screen and search for HotPlayer using the Siri Remote.
tvOS allows no sideloading at all, so HotPlayer must be published on the tvOS App Store — the iPhone version being available does not mean the TV version is.
Hold the TV button to return to the Home screen if HotPlayer stops responding, rather than unplugging the box.
